ORDER  FORM RENTAL  and/or  PURCHASE

 

The following Order Form is part of the Rental‑Purchase Agreement  between our Clients, herein called the RENTEEs, and Teb-Mar Products Inc., herein called the RENTOR.  The information provided becomes an integral part of the Rental‑Purchase Agreement  that will enable you to rent and/or purchase Teb-Mar Products Inc.’s specialized tools, the DrillJack-II ®, and the AnchorJack-II ®, herein called the TOOLS.

 

All rental and/or purchase requests require that the RENTEE : (1) reads, and accepts, the Rental‑Purchase terms & conditions, and then (2) completes the following form.

All fields with * are mandatory.  Only order forms duly completed can be processed in a timely manner.

Once you have completed filling in the necessary information, please print out the form and fax it to  819-671-2864, along with a covering letter, on your company letterhead.

 

The processing of your order will be done manually by Teb-Mar Products Inc. staff.

The ULTIMATE TOOLS have become very popular.  Some of our clients are finding them so efficient, and safe, that they are clamouring to purchase these Ultimate Tools for continuous use.

In reply to such requests, Teb-Mar Products Inc. has decided to make their Ultimate Tools available for purchase.

The following prices apply as of this date* :

  • the comply to the Rental-Purchase conditions that is automatically e‑mailed to us when you complete the requested information at the end of the Rental-Purchase contract's terms and conditions web page, and;

  • a copy of the relevant Insurance Certificate (a faxed copy will do)  -  on Rental orders only, and…

  • the appropriate Security Deposit  -  for Rental orders only.
